Role Description

We are seeking an accredited Home Medicines Review (HMR) Pharmacist to join our growing network on a flexible, contract basis, with the ability to conduct in-person consultations within your preferred local area.

You will receive GP-referred patients through the Chemist2U platform and be responsible for:

  • Conducting comprehensive medication reviews (HMRs)
  • Liaising with patients for in-person consultations
  • Preparing structured, high-quality reports using provided templates
  • Collaborating with GPs and healthcare teams to optimise patient outcomes
  • Providing practical, patient-centred medication advice to improve safety and adherence

Qualifications & Requirements

  • Current registration with AHPRA as a pharmacist
  • HMR accreditation (or eligibility to obtain)
  • Strong clinical knowledge of pharmacology and medication management
  • Experience in conducting patient consultations and education
  • Excellent communication and report-writing skills
  • Comfortable using digital tools for telehealth and documentation
  • Ability to work collaboratively with GPs and healthcare providers
  • Experience in community or remote pharmacy practice (desirable)
